Each year on the anniversary of the assassination of the tsar Alexander II, Timofeyev a mental patient, develops a sympathetic wound or illness. He is convinced that this is retribution for the fact that he was the one to actually commit the crime. Fascinated with his patient condition, Smirnov, his doctor, convinces Timofeyev to actually take on the identity of the killer. Smirnov gets lost in the charade and begins to assume the identity of the Tsar himself. The spell ceases to be broken until the two of them relive the events of July 1918.
